Your Doctor, In Time.

Dr. Brian Juan MD, FAAFP

A Primary Care Doctor in Longmont, Colorado

Dr. Juan is a board-certified family physician and founder of Metronome Family Medicine, a Direct Primary Care practice in Longmont serving patients throughout Boulder and Weld Counties who are seeking a primary care doctor in Longmont with continuity and depth.

He built his practice around a simple belief: medicine works best when physician and patient have the time and space to think clearly together.

His work has always centered on relationship-driven, evidence-based primary care — caring for individuals and families across seasons of life, from prevention to complex metabolic and hormonal transitions.

He trained at the University of Colorado School of Medicine and completed residency at Southern Colorado Family Medicine before spending nearly a decade practicing outpatient family medicine along the Front Range.

Board-certified in Family Medicine, he maintains a steady commitment to clinical rigor, continuing education, and thoughtful, systems-based care.

A Different Way to Practice Medicine

Over time, Dr. Juan recognized the constraints of high-volume systems: shorter visits, longer waits, and limited opportunity for comprehensive oversight.

The science of medicine remains strong.
The structure surrounding it often limits its full expression.

He believes effective primary care requires more than protocols and algorithms. It requires judgment. It requires continuity. It requires listening carefully enough to recognize patterns over time.

Time builds relationship.
Relationship builds understanding.
Understanding improves decision-making.

Direct Primary Care provides the structure that allows him to practice with depth, longitudinal oversight, and clinical accountability.

Continued Growth and Clinical Curiosity

Dr. Juan’s own experience of midlife has informed his clinical perspective.

As metabolism shifts and recovery evolves, broader questions of longevity, resilience, and sustainable performance naturally emerge. In his view, primary care should extend beyond disease management to support long-term strength and functional capacity.

This interest has expanded his ongoing education into systems-based and integrative approaches, with focused study in metabolic flexibility, hormone physiology, cardiometabolic risk reduction, and health span preservation.

This exploration does not replace evidence-based medicine — it builds upon it.

He remains grounded in the science of medicine, evaluating emerging research thoughtfully and applying what is well-supported within comprehensive primary care.

For many patients navigating similar stages of life, this shared experience fosters an added layer of understanding.

Curiosity, guided by evidence and experience, is what allows medicine to evolve responsibly.

Music and Medicine

Before his medical career, Dr. Juan was an internationally touring musician. He has written, recorded, and released original music for decades and has performed extensively throughout Colorado and beyond, including more than 20 years performing and recording with Otis Taylor.

The discipline of performance and the necessity of attentive listening shape the way he approaches patient care.

Rhythm matters.
Timing
matters.
Listening
is everything.

He also works with musicians and touring professionals seeking healthcare that adapts to life on the road.

In both music and medicine, precision and adaptability must coexist.

Professional Roles & Community

Dr. Juan serves as Clinical Faculty at the University of Colorado School of Medicine.

He is a recipient of the Golden Stethoscope Award for excellence in medical education and previously served on the board of directors for the Colorado Academy of Family Physicians.

He lives in Longmont with his wife and children.

Primary care, at its best, is built over years — not transactions. As a primary care doctor in Longmont, Dr. Juan values long-term physician–patient relationships and practices with the intention of caring for individuals and families across decades.

His goal is simple:

To provide the kind of comprehensive, thoughtful primary care he would want for his own family — and to practice in a way that remains steady, rigorous, and dependable over time.

Dr. Juan is THE BEST!! My husband and I have had many doctors over the years, all were...fine. Then we switched our insurance to KP and we found Dr. Juan. WOW. We were blown away at how wonderful he is! Here are couple examples that stick out for us:1. My husband and I have a son w/ an IDD, it has been the most difficult experience we have ever faced (uncertainties, fears, medical complexities, PTSD etc.). Never have we had any doctor show so much care and thoughtfulness, and true desire to help. 2. I was struggling with PTSD and depression among other things due to our situation. But something was still very off. For years people said it's mommy brain, until Dr. Juan. He actually listened and helped get an appt to further investigate. I was diagnosed with ADHD in my late 30s and it all made so much sense. Since then, it has been so much easier to function in my day to day. All thanks to someone FINALLY listening and genuinely caring. I can't say enough good things about Dr. Juan. We are so grateful!

  • Hundred stars Dr. Juan

Dr Juan is THE most thorough, thoughtful and knowledgeable doc I have ever seen. This man goes above and beyond EVERY single appointment. Makes you feel comfortable and takes time to make sure you understand everything about yourself, your health, any issues or concerns. If every doctor practiced medicine in this fashion, not one person would ever complain about their provider. I would give him a thousand stars plus. I followed him from one location to another as did my entire family. I’m afraid there are no others quite like him, if there were the entire Healthcare industry would be perfect, just like Dr Juan. An asset to his field.

  • Outstanding Expertise and Care

Dr. Juan is BY far the most thorough, nonjudgmental, collaborative doctor I’ve ever worked with. He’s been my primary care doctor for several years and has made a true difference in my life. Despite seeing many patients per day he still has a way of making everyone feel seen, and he explains things with enthusiasm, expertise, and care. I cannot recommend him enough.
