Direct Primary Care membership at Metronome Family Medicine covers your everyday primary, hormone, metabolic, and acute care — but it does not include true specialty care. The good news: a large share of what typically gets referred out in fee-for-service medicine can be managed directly, right here, by a physician who actually has the time to do it. And when a specialist really is the right call, we stay involved every step of the way.
Specialty Care Is Not Included in Membership
Metronome membership covers ongoing primary care, direct physician access, and the hormone, metabolic, and acute care detailed throughout this site. It does not include true specialty care — the kind delivered by a sub-specialist, in a specialist's office, using training or equipment outside the scope of primary care.
When a genuine specialty visit, procedure, or sub-specialist opinion is needed, that care is billed separately — typically through insurance, a health share plan, or directly out of pocket — the same as it would be anywhere else.

Fee-for-service medicine often refers out of necessity, not medical requirement: a 15-minute visit rarely leaves room to evaluate a skin lesion, adjust a thyroid regimen, or manage a joint injection start to finish. Direct Primary Care removes that constraint.

Routine skin exams, acne management, and in-office skin biopsies for concerning lesions are handled directly, without a dermatology referral in most cases.

Dr. Juan works to communicate directly with the specialist — physician to physician — both before and after your appointment, so your care stays integrated. You should never be in the position of feeling like your doctors don't talk to each other.
What Doctor-to-Doctor Coordination Looks Like

- Direct communication with the specialist's office when questions come up
- Results and recommendations reviewed and folded back into your ongoing care afterward
- One point of contact — your own physician — tying it all together
E-Consults: A Faster, Lower-Cost Alternative
In select cases, Dr. Juan can consult directly with a specialist electronically — sharing your history, records, and the specific clinical question — for a small fee that's a fraction of the cost of an in-person specialty visit.
Sometimes that e-consult is enough to answer the question and adjust your care without ever booking a specialist appointment. Other times, it sharpens exactly what that appointment needs to accomplish, so the visit itself is faster and more useful. Availability depends on the specialty and your specific clinical situation — ask during your visit whether an e-consult makes sense for what you're dealing with.
Your Needs Get Assessed Individually — Not by a Checklist
Nothing above is a promise that a given condition will never need specialist care. Every patient's history, complexity, and prior workup are different, and some situations genuinely call for a sub-specialist's training and equipment from the start. What you can count on is that the decision gets made with you, based on your actual situation — not a blanket rule about what Direct Primary Care does or doesn't refer out.
